Job Summary - CORRECTIONAL HEALTH
The Service Linkage Worker provides services within a broad range of focus areas to assist patients in accessing and adhering to care. Services provided may include HIV testing and counseling, patient assessments, assisting to reduce access and adherence barriers, follow-up to ensure referrals are completed, patient navigation assistance, and coordination with case managers and other clinic staff.
Minimum Qualifications
Education/Specialized Training/Licensure: Bachelor's degree from an accredited college or university with a major in social or behavior sciences. Documented paid work experience in providing client services to Persons Living with HIV/AIDS (PLWHA) may be substituted for the bachelor's degree requirement on a 1:1 basis (1 year of documented paid experience may be substituted for 1 year of college).
Work Experience: A minimum of 1 year paid work experience with PLWHA
Equipment Operated: Computer and general office equipment.

Other Skills: Analytical, P.C., MS Word
Work Schedule: Flexible, Eligible for Telecommute
Other Requirements: Completion of Texas Department of Health, HIV Case Management 101 within first 90 days of employment. Other required trainings as outlined in the Ryan White Standards of Care.
